All Managers, Coaches
and Assistants that represent the
Shelby Baseball and Softball Club
and their team must have an Application
and Release Form on file with the
Shelby Township Parks, Recreation
and Maintenance Department before
a team shirt can be issued for them!
This is a Township wide policy as
of the 2004 season. Any issues that
come up as a result of the background
check will be discussed only with
the PRM Director, Club President
and the League Commissioner.
These forms will be kept on file
at the Parks Recreation and Maintenance
Building. Commissioners will retain
a list of Managers, Coaches and
Assistants that have turned in
a copy. If you are unsure of your
(or your Coaches' or Assistant's)
status, contact your Commissioner.
If you have not filled out the
forms, fill them out and give to
your Commissioner prior to Uniform
Handout on April 19th. These forms
can also be obtained at the Parks
Recreation and Maintenance Office. |
